Liability and Risk Assessment Questionnaire

The Central Conference is inviting church leaders across our conference to complete this brief yet important Risk and Liability Assessment Questionnaire. Either the church chair or the lead pastor should complete this questionnaire. The Central Conference would appreciate receiving one response per church.

This tool is designed to help us better understand current practices, identify potential areas of vulnerability, and promote a culture of preparedness and care within our churches.

Information on, and a link to, the questionnaire has been emailed to each of the conference churches. If your church has not received this information, please contact the Central Conference office at 773-267-3060, or email info@centralconf.org.

Our goal is not to burden you, but to walk alongside you in safeguarding the flock God has entrusted to your care (Acts 20:28). By taking a few moments to complete this questionnaire, you will be contributing to a broader effort to ensure that our churches remain places of refuge, safety, and integrity.
